  Original & Photocopy of Form 138 from previous school (for elementary & high School)

  Certificate of Good Moral Character

  Records of Grades

  3 copies of ID picture (Passport size)

SEMINARY: Present the following:

   Requirements above, plus:

     Church recommendation signed by the Pastor  and Church secretary

FOR THOSE AVAILING SCHOLARSHIPS:

Any documents proving the applicant’s qualification for scholarship grant.

For Pastor, children and wife:

Church certification signed by the Church board or secretary

Recommendation from denomination Head or another Pastor.
